Summary

This is a full time opportunity offering a 4x10 work schedule - Wednesday, Thursday, Friday & Saturday from 10am to 8:30pm.

The Case Manager RN is a vital member of the multidisciplinary patient care team who applies clinical expertise, evidence-based criteria, and professional judgment to support safe, efficient, and patient-centered care. This role integrates utilization management, quality management, discharge planning, and resource coordination to promote optimal health outcomes and responsible stewardship of clinical and financial resources.

The Case Manager proactively predicts, facilitates, and evaluates transitions of care across the continuum, linking patients and families with appropriate resources, services, and opportunities to achieve and maintain their highest level of independence and functioning. By serving as both a patient advocate and a system navigator, the Case Manager ensures a seamless, collaborative, and compassionate care experience.

Key Responsibilities
  • Collaborate with physicians, nurses, social workers, and other team members to develop and implement individualized care plans.

  • Apply established medical necessity criteria to support appropriate utilization of services and timely progression of care.

  • Coordinate safe, efficient discharges by assessing patient needs, identifying barriers, and connecting patients with community resources.

  • Serve as a liaison between patients, families, payers, and providers to ensure continuity of care and resolution of issues.

  • Monitor, evaluate, and document patient outcomes, using data to drive quality improvement initiatives.

  • Educate patients and families on care plans, community resources, and self-management strategies.

  • Support compliance with regulatory requirements, accreditation standards, and hospital policies.

Requirements

Education

  • Associates Degree in Nursing required; BSN strongly preferred (or actively in progress).

Experience

  • Minimum 3 years of recent clinical nursing experience required.

  • Experience in case management, discharge planning, utilization review, or care coordination preferred.

  • Knowledge of community resources and social/clinical factors affecting patient outcomes strongly desired.

License

  • Current, unrestricted RN license in the State of Florida.

Certifications

  • Case Management certification (ACM, CCM, NACCM, or CMCN) required within 5 years of employment.

  • BLS certification may be required per facility standards.

Skills & Competencies

  • Strong clinical assessment and critical thinking skills.

  • Excellent communication, collaboration, and interpersonal abilities.

  • Proficient with EMR systems and basic computer applications.

  • Ability to prioritize, multitask, and manage time effectively in a fast-paced environment.

  • Demonstrated commitment to patient advocacy and continuous performance improvement.
